Description

This position to provide case management services for individuals with alcohol/drug issues, serious mental illness, and addiction issues. Other duties include providing treatment and rehabilitation services through a multi-disciplinary team along with assisting in developing and maintaining quality emergency mental health and addiction services; providing consultation to community organizations regarding addiction and behavioral health topics and services.

Performs intake, assessments, intervention, individual, group and family counseling, outreach referral, and advocacy; follow up care; provides educational and preventive activities. Assists in planning, developing, organizing and facilitating services/programs such as support and therapy groups, outpatient, prevention activities and special events. Obtains information about patients through interview, case history, and observational techniques; evaluates data to identify causes of problems and to determine advisability of counseling or referral to other resources.Develops program for treatment and rehabilitation of client, using knowledge of alcohol and drug abuse problems and counseling and treatment techniques. Provides and/or arranges for therapeutic interventions as appropriate for patients or clients in a crisis condition. Provides information, education and support family members to assist family in dealing with providing support for client. Assesses and manage situations that require immediate attention including walk-in clients, clients in need of detoxification services, emergency inpatient evaluations appointments for residential services funding and emergency telephone crisis encounters. Maintains accurate documentation relevant to cases, including assessments, recovery plans, relapse prevention plans, progress notes and discharge summaries. Attends and arranges staffing and team meetings to ensure quality care and maximization of resources. Keep program manager apprised of consumer progress, needs, and any critical issues. Provides consultation to other units within DHHS and to the community regarding mental health and addiction services. May participate in county Recovery Courts.Attends court hearings and provides expert testimony as required.Collaborates with other sections of DHHS to ensure effective services for consumers and the linkage of the consumer to economic, health, and family support services.Refers client to other support services as needed, both internal and external. Coordinates counseling efforts with mental health professionals and other health professionals. Documents assessments, treatment plans, reviews, referrals and discharge planning and summaries in accordance with professional standards, agency, state and federal guidelines. Bills all allowable services and submit billing as directed. Ensures state reporting system forms and data collection are completed on a timely basis. Other duties as assigned.
