We're looking for a Community Support Care Professional to join our team. A successful candidate has previous experience working with the developmentally disabled, a strong work ethic and an active interest in helping to care for our consumers. It is often said that there is no greater satisfaction than working directly with our consumers, and this is absolutely true. It's extremely rewarding, since many of our consumers are living well.

- Professional-level position accountable for implementing, monitoring and reviewing individualized habilitation plans for 8 to 16 consumers with developmental disabilities
- As part of an interdisciplinary treatment team, observing behavior and participating in habilitation programs and SOP/ISL Homes.
- Preparing reports and progress notes, assisting with annual staffing, and completing quarterly and monthly reviews
A Bachelor's degree or 4 or more years of relevant experience. (Substitutions may be allowed).
