Essential Job Duties:

The Program Specialist II with the Texas Correction Office for Offenders with Medical or Mental Impairments (TCCOOMMI) works with a small team doing work that makes a huge impact. The Program Specialist II¿s role is toidentify and screen for assessment or immediate referral to appropriate connection for ongoing case management and planning. This position works with many agencies and state data bases and reads and evaluates reports in order to perform identification processes. The Program Specialist II is a decision point position in continuity of care services within TCOOMMI programs and requires a mind for critical thinking, innovation, data management and attention to detail.

TCOOMMI program services are vast and resources are spread across the state, this role must effectively communicate needs of clients/inmates for appropriate warm transfers and assist in the oversight of the continuity of care flow identifying issues or trends that could impact operations or program intent.
