Community Transitions Navigator, Saluda/Edgefield Full-TimeOn-siteLocations Showing 1 locationSelf Regional HealthcareGreenwood, SC 29646, USADescription 1st Shift 8am-5pm Monday-FridayThe Community Transitions Navigator (CTN) provides healthcare navigation services to enrolled clients/patients, coordinates medical home placements and first appointments, collaborates with a multi-disciplinary team to address the social determinants of health and arranges supportive services and referrals to community partners for improved health outcomes and to prevent avoidable hospital admissions/emergency department visits. CTN may provide limited health coaching and advocacy to improve assigned clients'/patients' appropriate healthcare utilization and may assist them with applications for healthcare payor options, prescription assistance and/or other benefit programs.
